Before you begin your CE Provider application, be sure you have the following available:
- Information about your organization (name, address, EIN)
 - Contact information for a primary point of contact for your organization
 - Supplemental materials (view this chart for the requirements specific to your organization type)
 - For each CE program offered:
	
- Program title
 - Description (view samples PDF)
 - Target audience (e.g., EA, ERPA)
 - Category (e.g. ethics, federal tax/tax related matters, etc.)
 - Delivery method (e.g., in-person, self-study, online)
 - Number of CE hours received
 
 - Credit card, Paypal or bank account payment information to process your non-refundable $650 application fee
